Who can file an LUT?

Any registered exporter of goods or services, other than one prosecuted for tax evasion exceeding the prescribed amount.

How often must it be filed?

Once for each financial year. It does not carry forward automatically, and exporting without a valid LUT means paying IGST and reclaiming it.
